Sunny Dale Service Center provides person-centered services that help individuals live, learn, work, and participate in their community with dignity and support.
Every person we serve has different needs, goals, interests, and abilities. Our services are designed to provide practical support while encouraging independence, confidence, community involvement, and quality of life.
Community Living Supports
Community Living Supports provide assistance to help individuals live safe, productive lives in their home and community. These supports may be provided in an individual’s own home or family home and are based on the needs and preferences of the individual and their family.

Daily Routines
Community Involvement
Building Independence At Home
Community Access Individual
Community Access Individual services provide one-on-one support that helps individuals express their talents, explore their communities, and participate in meaningful activities. These services are delivered with a person-centered approach and are focused on building confidence, promoting growth, and encouraging independence.
Community Access Group
Community Access Group services help individuals acquire, retain, or improve the skills needed for active participation and independent functioning. These services are provided in a group setting with peers and may take place in both facility-based and community-based settings.

Social skills
Community participation skills
Self-help skills
Confidence in group settings
Greater independence outside the home
Community Residential Alternative services provide 24-hour personal support for individuals living as roommates. Support may include assistance with medication administration, health and wellness oversight, household maintenance, cooking, money management, shopping, recreation, and socialization.
The goal is to help individuals live full, meaningful lives in a safe and supportive residential environment.

Sunny Dale Service Center provides person-centered services that help individuals live, learn, work, and participate in their community with dignity and support.
Supported Employment
Supported Employment services assist individuals in obtaining and maintaining competitive, integrated employment in community settings. This may include on-site job training, performance monitoring, skill development and retention support.
